Documents Needed – Initial Arrangement Conference with the Funeral Director

 

Insurance Papers (if needed):

  1. Policy Number(s)
  2. Company Name, Address, and Phone Number
  3. Beneficiary Information

 

VA Forms (if needed):

  1. DD214 (discharge papers)

 

Information for Death Certificate

  1. Full name of deceased
  2. Address of Deceased
  3. Date of Birth
  4. Social Security Number
  5. Name of Attending Physician
  6. Educational Background (highest grade attained)
  7. Father’s Name:
  8. Mother’s Maiden Name
  9. Marital Status
  10. Occupation During Most of the Deceased’s Working Years
