  3. Beneficiary Information

 

Information for Death Certificate

  1. Full name of deceased
  2. Address of Deceased
  3. Date of Birth
  4. Social Security Number
  5. Name of Attending Physician
  6. Educational Background (highest grade attained)
  7. Father’s Name:
  8. Mother’s Maiden Name
  9. Marital Status
  10. Occupation During Most of the Deceased’s Working Years

 

VA Forms (if needed):

  1. DD214 (discharge papers)

 

 

Information To Gather As You Plan The Service (To Be Given to the Funeral Director)

Program Information:

  1. Obituary
  2. Order of Service Participants (Clergy, Choir/Soloists, Tributes/Reflections, etc.)
  3. Poems/Bible Verses/etc.
  4. Photo(s)

 

Information for Newspaper Notices:

  1. Names of newspaper(s) that will run the notice

 

Clothing:

  1. Outer garments
  2. Under garments
  3. Socks/Hosiery (shoes are optional)
  4. Glasses (if needed)
  5. Jewelry/Special keepsakes (if needed)
  6. Hairpieces/Wigs (if needed)
  7. Make-Up (if needed)
